Below is Cornerstone’s “Notice of Privacy Practices” (“Notice”). The Notice explains how we meet this commitment. The Notice also explains what is in your health record and your rights under federal and state laws. All people and places that make up Cornerstone must follow the Notice. However, this does not include Cornerstone as an employer. Each has its own notice. Additionally, if your doctor is not a member of a physician practice that is owned by Cornerstone, they may have different policies about how to handle your personally identifiable information and will have a separate Notice.

If you have a question about your health information in this summary, you should review the full Notice of Privacy Practices or ask a Cornerstone staff member for more information. Information That We Collect
You may use our website without providing any personal information. When you reply to any of our communications with a request, we may need to collect additional information. Patients can opt to create a Patient Portal account. Patient Portal is a secure portal available for patients to manage their health. Features include reviewing test results, managing appointments, messaging the office. Before creating an account, please review the Terms and Conditions available on the Patient Portal website.
Cornerstone Care is committed to protecting the privacy of the personal information you provide to us via this website so that we can make sure it remains as secure as possible. Accordingly, we use reasonable efforts to prevent unauthorized access. We use a secure firewall and a security infrastructure that protects the integrity and privacy of personal information submitted to us via this website.
Although we work hard to protect your personal information, Cornerstone Care cannot guarantee the security of any information you transmit to us through online applications, and you do so at your own risk. If you receive an email that looks like it is from Cornerstone Care and asks for your personal information, do not respond. This may be a scam or “Phishing” email intended to steal your information. Cornerstone Care will never request your password, username, credit card information or other personal information through email.
