Even with expanded health insurance and the Affordable Healthcare Act, there are still those without coverage or who may not be able to afford their medications due to high copays, deductibles or other reasons.

Cornerstone Care’s Prescription Program makes many prescriptions more affordable by offering pricing discounts through local pharmacies. Even if you have prescription insurance and have no difficulty affording your medications, by choosing one of our partner pharmacies, you can help us help those who do. And for those times that your insurance might not cover your medications, our discounted prices can help you, too. Simply choose one of the pharmacies listed below, let them know you are a patient of Cornerstone Care and the pharmacy and Cornerstone can take care of the rest.

What is the Healthy Woman Program?

Adagio Health is the statewide administrator of the Pennsylvania Department of Health’s Healthy Woman Program which focuses on early detection of breast and cervical cancer.

Adagio Health is the sole provider of Healthy Woman programs for more than twenty counties of western Pennsylvania. Through HealthyWoman, eligible women can receive free breast and cervical cancer screening.

As a companion to Healthy Woman, Adagio Health also offers clients a cardiovascular health program called WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across the Nation) at selected locations. WISEWOMAN targets low-income, underinsured or uninsured, women ages 40 to 64 and provides them with the knowledge, skills, and opportunities to improve their diet, physical activity and other life habits to prevent, delay, or control cardiovascular and other chronic diseases.

A proud partner of Susan G. Komen Pittsburgh, Adagio Health administers Komen Pittsburgh’s 34-county Mammogram Voucher Program which provides mammograms for uninsured and underinsured women. Komen’s Mammogram Voucher Program (MVP) became a partner, providing additional resources for:

  • Screening mammograms for all women 40 and over who cannot afford them
  • Diagnostic services for all women in the program identified with breast abnormalities
  • Educational forums about breast health to be provided throughout the region
  • Access to the Internet through the Breast Cancer Information Service (BCIS)
  • Opportunity to participate in Pennsylvania’s largest foot race—the Mother’s Day 5K in Pittsburgh

In addition to cancer screening, Adagio Health sites provide follow-up and referral.

Why it's critical to your good health

Diabetes education is the cornerstone of diabetes management, because diabetes requires day-to-day knowledge of nutrition, exercise, monitoring, and medication.

Diabetes is different from other diseases that medication alone can often times successfully treat. There are many components to diabetes, such as: the diabetes disease process, nutritional management, physical activity, medications, glucose monitoring, and psychosocial adjustment.

Diabetes education makes you more aware of diabetes, what it takes to treat it, and gives you the power to control it. Diabetes education allows you to better incorporate education into your life and make the necessary changes to improve your lifestyle.

Diabetes education and self-management training should be done with a team, through an integrated system of care, such as that provided by Cornerstone Care.

Your diabetes care team consists of:

  • Your Primary Care Physician who understands diabetes
  • A certified Diabetes Educator
  • A Nutritionist or Counselor
  • A Podiatrist
  • Your Dentist

Cornerstone Care’s integrated system of care gives you access to all of these people.

Your Cornerstone Care primary care provider will help you with your diabetes self-management goals, which will be individualized, measurable and achievable. They will help you create very specific short-term goals as well as long-term goals. An example of a short-term goal might be saying you’ll lose one to two pounds per week, while a long-term goal would be to lose 50 pounds.

Diabetes education is an ongoing process that should be repeated every one to two years. This allows patients to keep up to date. Diabetes education gives you the power to control your diabetes, so it doesn’t end up controlling you.
