Cornerstone Care Community Health Centers is are proud to announce the implementation of a new program called “Smile Care”. This program will provide payment assistance for those patients who have established a dental home with any Cornerstone Care Dental Facility including the Mobile Unit. Cornerstone Care has been awarded funding from the Delta Dental Community Foundation to provide services for eligible uninsured and underinsured dental patients to help remove barriers to care such as lack of insurance and inability to pay.
CORNESTONE CARE SLIDING FEE SCALE AND THE SMILE CARE PROGRAM
The Smile Care program works in unison with Cornerstone Care’s existing sliding fee scale. All patients should apply for the sliding fee plan. The sliding fee program provides patients an opportunity to reduce their fees based upon their income. Even insured patients should apply for the sliding fee plan to determine eligibility to receive reduced co-pay and deductible expenses for Cornerstone Care dental services. Discounts range from a nominal fee charge of $25.00 to a 60% discount on charges. Those patients, who can provide documented proof of being unable to afford the nominal fees, may apply for a fee waiver.
WHO IS ELIGIBLE FOR THE SMILE CARE PROGRAM?
All patients who wish to apply for consideration under the “Smile Care” program must complete a Smile Care application form. Program Financial Assessment must be recommended by a Cornerstone Care dentist for all patients seeking dental treatment at a Cornerstone Care Dental facility. If approved for the “Smile Care” program, the guidelines provided for program assistance will determine patient benefits. If approved, most patients will pay little to nothing for the general dentistry services approved under the program. Only applications that have all required signatures, information and documentation will be considered.
Typically, patients who qualify for the nominal fee are not eligible for the Smile Care program; however, there are exceptions such as homelessness; nominal fee waiver approval and children under the age of 18.
HOW DO YOU ESTABISH ESTABLISH A DENTAL HOME WITH CORNERSTONE CARE TO EB BE ELIGIBLE FOR THE SMILE CARE PROGRAM?
You must schedule and complete a preventative dental visit and receive a dental treatment plan provided by a Cornerstone Care dentist within five months of submitting a Smile Care application. All services must be provided under the Cornerstone Care dentist recommended treatment plan.
WHAT SERVICES ARE COVERED?
Orthodontics and cosmetic procedures are NOT COVERED under the Smile Care program.
ARE THERE LIMITS ON PROGRAM BENEFITS?
Yes, there are annual limits on all services except preventative and restorative care for youth under the age of 18.
PROGRAM LIMITATIONS
Benefits under the Smile Care program will only be available as grant funds are available.
